"I understand" is present tense, and "I understood" is past tense. Normally, when replying to someone who has given you an explanation or instructions, you would use "I understand," because your understanding is something that is happening in the present (no matter when it started).
"I understood" would be used for situations that happened entirely in the past and are essentially finished (although you technically continue to understand in the present, because you haven't forgotten about it): "I understood what he wanted, and got him the book he was looking for."
